The facility is experiencing a Covid breakout. Staff is doing a great job with mask. I would feel better if they told you up front that this was going on.

My grandmother was admitted to Grandview a couple of weeks ago. I have nothing but good things to say. We first met Kim at the hospital and she was very kind and helpful. After Grandma was admitted we met Teresa, the nurse she was so kind and made sure that we had everything that we needed. Amanda from admission was wonderful, after sitting down with us and answering all of our questions. She asked was there anything else that she could do, we asked for her to get someone to get some ice water. She took the time to get the water herself. One of our favorite night shift nurses was Jasmine. she made me feel better when I left at night knowing that she would be there. I can’t name all the CNAs that we came in contact with but they were great. Last but not least the therapy department, grandma was very weak when she went in but with the therapy she can now walk with a walker. I’m so happy that we chose Grandview and if ever needed we will be back again.

This facility was amazing to my late husband, Fred! Poor thing was demented and could hardly walk, but by golly, Grandview Post Acute made his final days on earth comfortable and easy. Uriah and his staff were attentive and listened when we had concerns. Amber and Phyllis were both lovely to chat with and helped whenever they could. The negative reviews about this place were when people worked there who didn’t do their job. Those people have since been dismissed. Grandview is now better than ever and running much more smoothly.

I was there for a week in June and it was the worst experience I’ve ever had. Nobody address the fact I had a bad hole in my head and they ignored it until it was very infected. Mitze was the best nurse there
Amazing facility! The staff are caring and go above and beyond for their residents.

Since the new ownership in Dec 2024, this facility has been improving with leaps and bounds. The administrator truly cares about the wellness of the staff and residents. Yes, there is room for improvement but there has been so many positive changes that I can only look forward to future days.

My mother in law was at Grandview for a rehab stay. During this time my wife and I were in and out at different times of the day. We saw many opportunities for improvement facility and staff wise, but want to specifically call out Whitney Tayes. Whitney was very instrumental in helping my mother in law get better. One can tell that this is not just a job for Whitney, but those patients on the hall are her people. One instance that stands out was that there was a difficult patient across the hall and when there was an issue Whitney handled it with patience and compassion. She is truly one in a million!

First, I want to acknowledge that the staff members I encountered were kind on a personal level. However, beyond that, this facility is absolutely unacceptable and, in my opinion, should be shut down. My grandmother was admitted on Friday, and I went to visit her last night. From the very beginning, there were serious issues: Entry Issues: We waited 15 minutes just to be let into the building. In the end, we had to ask another visitor who was leaving to find someone to unlock the door. Safety Concerns: When we reached my grandmother’s room, a man in a wheelchair entered, blocked my daughter and me in a corner, and said, “How are you going to get out now?” While my son ran to get a nurse, this man began hitting my grandmother’s feet. Keep in mind, my grandmother was actively dying at this point (she passed away the following morning). If we hadn’t been there, this man could have seriously harmed her with no one knowing. Before we left, the same man came back into her room again, and the nurses had to remove him. Lack of Medical Care: My grandmother was crying out in pain, so I went to the nurses’ station to request pain medication. The only thing they could give her was Tylenol—because a physician had never once seen her during the three days she had been there. She was 86 years old, in kidney failure, and left without a single physician evaluation. Recordkeeping and Medication Delays: When my cousin asked for her room number, staff didn’t even have her in the system. My cousin demanded she be given appropriate pain meds, as by then my grandmother was vomiting fluid. Finally, they managed to get a physician on the phone, who prescribed her medication. But since their pharmacy is located in Nashville—and it was a Sunday—her meds couldn’t be obtained. This left my grandmother in severe, unmanaged pain. Our family refused to leave her alone, so my cousin stayed with her through the night. I’m grateful he did, because my grandmother passed away soon after. She died in pain due to this facility’s negligence. I have dealt with nearly every nursing home in this town through my late mother’s care, and I can honestly say this is the worst one I have ever experienced. The only reason my grandmother ended up here was because it was the only facility that accepted her insurance. Please—do not leave your loved ones in this place. No family should have to go through what ours did. At least my grandmother is no longer suffering and does not have to endure Grandview any longer.

Subject: Concerns and Appreciation Regarding Barbara’s Care Dear Grandview Team I am writing to share both gratitude and serious concerns regarding the recent care of my grandmother, Barbara, who was admitted to your facility on Friday afternoon. First, I want to express my appreciation. From the moment I arrived around 6:15 p.m. that Friday, I noticed how clean and welcoming the facility was. The nurses and CNAs were consistently kind, attentive, and professional. The building did not have the typical “nursing home smell,” and every staff member I encountered—especially the gentleman nurse on Sunday—was compassionate and responsive. Please extend my thanks and kudos to your support team for their excellent work. However, I must also share some serious concerns: Front Desk Check-In: Upon arrival Friday evening, the front desk attendant could not locate my grandmother in the system and simply told me to “go check the room number” provided by my aunt. While I did find my grandmother, this lack of verification felt unsettling and did not inspire confidence in visitor management or patient safety. Weekend Medical Care and Pain Management: On Sunday, my grandmother’s condition worsened, and she was in significant pain. The male nurse promptly contacted the nurse practitioner for orders to ease her discomfort. Unfortunately, I was later told that the prescribed medication would not arrive until the next day because the pharmacy is located in Nashville and that the nurse practitioner “doesn’t even know her case, from a different nurse. As a result, my grandmother endured nearly 48 hours with no relief beyond Tylenol and could not even receive Zofran for nausea. We were further informed that my aunt could not meet with a doctor until Monday after 11 a.m., meaning my grandmother would have suffered almost three days with only minimal care. While I understand weekends present staffing and logistical challenges, it is distressing and unacceptable that a patient in severe pain would have to wait so long for proper medical attention. This represents a serious gap in your weekend medical coverage and pharmacy procedures. I respectfully urge you to review your protocols for after-hours medical care, pharmacy access, and communication between nurse practitioners and weekend staff. No family should have to watch a loved one suffer in pain without appropriate relief. Again, I deeply appreciate the kindness and dedication of your nurses, CNAs, and support staff. They did everything they could within their limits. My concerns are directed toward the systemic and administrative processes that left my grandmother without adequate care. Thank you for your time and attention to this matter. I look forward to hearing how you plan to address these issues to prevent them from happening to other families. I sent and email through your contact portal so you have my contact information. Barbara passed in her sleep at 2:30 AM on Monday morning peacefully but without any pain management and nobody would even know she passed unless my cousin had not stayed the night with her because we knew something was happening. Sincerely, Concerned grandson
